Subject: Revised Order relating to the fatal / ordinary accident and disposal of fire and investigations and reports relating to power installation under section -161 of Electricity Act 2003.

The procedure was determined in relation to the dispatch of the test report of the electrical accident in government order no. -1009 FP-2 / 91-23 dated 17-12-1991 (photocopy attached). Instructions have been issued to you and officers of the Directorate, after scheduling the timetable under the headquarter's No. 5-57-MU / 0 Accident / Normal date dated 29-08-2005 (Photocopy is attached). There is a possibility of an increase in electrical accidents due to the increase in the electricity system currently. There is a need to provide immediate help to the persons suffering in the accident, due to which the need to check the accident and then thereafter need to reduce the deadline to send the investigation reports of the concerned management directors. Therefore, the purpose of sending the inquiry report, while revising the order no. 567-MU / V. Accident / General dated 29-08-2005 issued by the Directorate, is re-simplified and according to the following procedure, investigate the accident and report To dispatch you are given orders to the concerned Investigation Officer of the Directorate: -

1- In the zonal office, any source from which the TV / news letter is also included, the result of the incident / fire after the incident of the fire, will be nominated by the Assistant Director on the same day to be nominated as an inquiry officer and fixing the date of the inquiry The information related officer / person will be transmitted in such a way that it can be found within two days. Phone / fax / e-mail should also be used to inform the date of the inquiry and the information should also be sent at the local level by sending the letter to the carrier. Generally, electrical accidents will be investigated by the Assistant Director, but if required, normal electrical accidents / firecrackers can be examined by the electrical inferior engineer in their supervision.

The task of investigation of each accident / fire brigade should be completed within 7 days from the date of receipt of the information of the accident by the zonal office. On the date of receipt of full details of the fatal accidental person, the concerned District Magistrate will be requested to get the police report.

Due to the presence of the passenger bus / tractor trolley overhead lines in connection with the wires or due to the inadequacy of the overhead line of the conductor, or the case of accidental hanging on the pole while working on the line, the sensitive case There is a possibility of resentment in public opinion. Therefore, on receiving the information of the accident in such sensitive cases, on the same day, the site has been examined by the Assistant Director and on the basis of available evidence, a primary (preliminary) report should be prepared and forwarded to the principal and regional office on the same day and on the telephone Also, let the Deputy Director be informed in detail so that the Regional Office will send letters to the headquarters and the administration / administration. The Regional Deputy Director will also give special instructions to the concerned Assistant Director of the level in the above mentioned sensitive cases, in writing / in writing, and will be giving written and verbal information to the director while doing the monitoring work.

2- After completion of the investigation work, after completion of the investigation work, the task of issuing orders under the rule 5 (4) for redressing the defects / errors found in the completion of the detailed report and the installation and demand of the pool / medical report etc. After doing this, make sure to record the report of all the documents, by submitting a brief note of the column 9 of the Assistant Director, Form No. 4, Send L'Office more than in three days.

3- Do not take more than 10 days to send to the Regional Office by completing the task of accident investigation at Zonal office level. If any indispensable reason is spent more than 10 days, the reasons for delays with proofs / proofs should be attached with separate sheet in duplicate and sent to the Regional Regional Office with the inquiry report.

4- With the Zonal Officer's inquiry report along with the cover letter (attached to the photocopy of the form) and nine-nine copies of the form no. 4 (express and readable) to be transmitted to the families of the different officers / victims, by typing type / computer type Attach with The photocopy of the cover letter related to the area being done by the electricity distribution franchisee in the district / city (like Torrent Company in Agra) is attached.

5- The Regional Headquarters, by the zonal office, each of them should be captured in each condition by the Regional Office on the second day of the dispatch date or dispatch date.

6- It may be ensured to send the Regional Office to the Regional Office by the other zonal offices subordinate to the Regional Office in such a way that in every condition the area is received within 05 days. The associated assistant director will be liable for the bill that takes more than 05 days.

7- On receipt of the inquiry report from the zonal office, after thorough examination of the Regional Sub-Declaration report, in the form of the Directorate, on the cover letter, by mentioning the prefix / recommendation in column 10 of the form no. Within 5 days of signing, the concerned Managing Director, (If the Managing Director's office is in another district) General Manager ( Aus) Power Corporation Limited, Lucknow Head Office and the relatives of the victim / accident send directly by post. If the office of the Director General, in which the Regional Office is located in the district, the postal reports will be forwarded by the correspondent, the copy of the Superintendent Engineer and the Executive Engineer of the corporation will be sent through the zonal office.

If there is any deficiency in the report, then the deputy director indicating the errors / shortcomings within the next five days will return the assistant director to the amendment order, fixing a certain time according to the nature of the amendment work.

8- If the zonal office is delayed at the level of the investigation, then the deputy director on his attached note / affidavit, on the attached reason, in respect of the irregularities in the investigation work, the letter written to the Assistant Director and all other correspondence related to the case, Attached with a copy of the original papers of a copy, a copy of all the information points on the concurrent proceeding form (form attached) Will forward the letter to the Head Office. If the Deputy Director is not satisfied due to the delay reported by the Assistant Director, then he will recommend strict action against them. The attached letter should be attached with a copy.

9- If the time has been more than the prescribed 05 days in the sending of quantity in the Regional Office level, then the head of the delay should be sent along with attachments to the head office by placing it on separate sheet (including authenticator).

10- - The Deputy Director will send the monthly statement (on attached format) to the head office by the 05th of the following month for the related disasters related to each month.

Please ensure that the action is taken with immediate effect according to the procedure prescribed above.
